In music, an accidental is a note of a pitch (or pitch class) that is not a member of the scale or mode indicated by the most recently applied key signature. In musical notation, the flat (♭), natural (♮) and sharp (♯) symbols, among others, mark such notes - and those symbols are also called accidentals.

However not all incidents can be termed as an accident.

Physical examples of accidents include unintended motor vehicle collisions, tongue biting while eating, electric shock by accidentally touching bare electric wire, drowning, falls, being injured by touching something sharp or hot, or bumping into something while walking.

2. Location.
3. Full names of people involved.
4. Names of witnesses.
5. Events leading up to the accident.
6. Environmental condition if applicable (slippery floors, poor lighting, hazardous materials etc.)
